Akutes Lungenversagen

Das schwere akute Lungenversagen (acute respiratory distress syndrome, ARDS) ist durch eine schwere und anhaltende Störung des pulmonalen Gasaustausches mit konsekutiver Hypoxämie und gleichzeitig bestehender pulmonaler Hypertonie gekennzeichnet.  

Die maschinelle Beatmung ist eine primär unterstützende Therapie bei Patienten mit einem akuten Lungenschaden (= ALI = acute lung injury), die erlaubt einen notwendigen Gasaustausch aufrecht zu erhalten bis die Ursache des Atemversagens korrigiert wurde. Obwohl die maschinelle Beatmung bei diesen Patienten eine lebensrettende Maßnahme darstellt, wurde früh erkannt, dass eine maschinelle Beatmung direkt über verschiedene Mechanismen einen Lungenschaden (Beatmungsinduzierter Lungenschaden = VILI; ventilator induced lung injury) induzieren kann. Diese wurden im Laufe der Jahrzehnte identifiziert:

•  Barotrauma = VILI aufgrund eines hohen Drucks

•  Volutrauma = VILI aufgrund eines hohen Volumens

•  Atelectrauma = VILI durch zyklisches Rekrutieren und Derekrutieren der Alveolen

•  Flowtrauma =  VILI aufgrund eines hohen Atemflusses und hoher Rate an Veränderungen des Lungenvolumens

Alle oben verursachenden Faktoren einer VILI stehen in Verbindung mit den Beatmungseinstellungen, obwohl sie auch während spontaner Atmung auftreten können. Klinisch und tierexperimentell untersuchen wir in unserem ARDS-Zentrum verschiedene Therapieoptionen und mit dem Ziel der Algorithmen für eine differenzierte Therapie zu etablieren.
